We checked CMS's math.
It didn't hold.

In July, CMS said about 12% of nursing homes nationwide qualify for its new Risk-Based Survey. The memo's own counts say 10.63%. Its national row averages 51 state rates, so Wyoming's 36 facilities weigh as much as Texas's 1,176.

Every commission is held to the same standard: the answer, the arithmetic, the source, and what the record can't tell you.
